Appointments and Payment

All appointments are at 130 W 5th St., inside the Leadville Post Office Bldg.

Tuesdays 3:30pm - 7:30pm

Fridays, 12pm - 5pm

(Last appointment scheduled 1 hour before closing)


Scheduling Appointments

We encourage patients to schedule online and in advance, however you are able to book appointments as available the day of. You may call or text to schedule if needed. 

New Patient Paperwork and Fee

New patients must complete the intake form and mandatory disclosure paperwork prior to treatment. Complete new patient paperwork onine here, or come 10 minutes early to your first appointment to complete in person. New patients are charged a one time $10 administrative fee in addition to your sliding scale payment ($25-$45). If this causes a hardship, please feel free to talk with us about it. 

Payment

Our sliding scale is $25 - $45. Pay what is comfortable for you, no questions asked! We accept cash, check, Venmo (@greatergoodacu) and Paypal. If you prefer, you can pre-purchase credits that will remain on your account (like a punch card). Let us know if you prefer to do this, and we'll get you set up!

Cancellation policy

Please cancel appointments 24 hours in advance to avoid a cancellation fee ($20). If you have an illness or emergency within the 24 hours window, please let us know and the fee may be waived.
